Addressing the Real Challenges with Aging

Declining health, isolation, and limited access to care aren’t inevitable parts of aging — they’re often symptoms of deeper systemic gaps. From income and geography to digital access and social stigma, these disparities shape how we age. True progress begins when we address these root causes together.

Working Together for Health that Lasts

We’re working to build healthier, longer lives for all..  We partner with communities, researchers, and leading healthcare systems to improve how we age —through smarter policies, stronger connections, and practical solutions that help older adults stay healthier, independent, and engaged in the places they call home.

Our Center for Healthy Aging (CHA) works to advance the health and well-being of aging populations through community-informed research, innovation in programming, and stakeholder coordination. CHA contributes to immediate and long-term systemic improvements to ensure optimal health for all older adults —  now and for generations to come.

We take a multidisciplinary approach to informing and shaping programs and policies that impact older adults and caregivers, including:

Conducting research grounded in needs and priorities identified through direct engagement with communities, older adults, caregivers, program implementers, service providers, and decision-makers.
Disseminating research findings and promoting a data-driven, evidence-informed approach to planning and decision-making to shape and strengthen city, state, and federal policy.
Providing evidence-informed strategic assistance for program delivery and policy implementation.
Convening stakeholders to exchange information and foster multisector collaboration, and developing creative programs at the intersection of public health, arts, and humanities to elevate key aging topics

Our Work in Action

Older Adults' Equity Collaborative
NYAM coordinates a network of ACL-funded organizations to expand access and equity in aging services. This initiative addresses disparities in health status by improving service delivery for older adults, especially those in underserved communities — critical work that directly impacts the proportion of seniors with better health outcomes.
Partnership to Support Aging in Place (PSAP)
As the Geriatric Technical Assistance Center for six New York State-funded programs, NYAM supported aging-in-place strategies for older adults impacted by COVID-19. Our strategic approach worked to enhance self-care capacity through integrated behavioral health and social supports, and helped reduce institutionalization and improve independence among seniors.

NYAM’s IMAGE: NYC tool visualizes aging-related health disparities, including chronic conditions like hypertension. By identifying geographic gaps in services and unmet needs, this data-driven platform informs targeted interventions and resource allocation to improve cardiovascular outcomes across diverse aging populations.
