ABSTRACT
Building on recent national and NYS health care reform initiatives focused on addressing the social determinants of health—such as housing, food security, jobs and education—this report illuminates factors for successful partnerships between health care organizations (HCOs) and community-based organizations (CBOs) that further these efforts.
The report describes partnering experiences, and outlines challenges and facilitators, as reported by representatives from NYC HCOs and CBOs during focus groups and key stakeholder interviews. The report provides actionable information for HCOs and CBOs seeking to enter into or enhance existing partnerships to support the needs of patients, clients, and communities.
