At the 4th annual Population Health Summit, “Working Across Sectors to Address Social Determinants of Health” co-hosted by the New York State Health Foundation, the New York Academy of Medicine and NYU Langone Medical Center’s Department of Population Health, health experts from across the region worked to deepen and extend the understanding of ongoing efforts in New York State and nationally that demonstrate effective collaboration between health care and other sectors to advance population health. 

Given the growing evidence that nonmedical determinants (such as housing, access to affordable foods, and opportunities for physical activity) have a disproportionate impact on a population’s health, the Summit focused on how the health care sector and New York State’s health care delivery reform initiatives have responded to address the social determinants of health with a wide range of activities that bridge health care and population health activities.
